Leon B. Howard, Jr.

“As a disabled war Veteran and as the Discharge Manager for a psychiatric facility, I have encountered first-hand the challenges that come with both being an applicant and assisting applicants with applying for benefits that they are entitled to. I believe that with the proper assistance and guidance, more eligible applicants can have many if not all their needs accommodated as well as services provided for their medical, social and psychiatric needs.” – Leon B.     After serving as the Discharge Manager of Northbrook Behavioral Health Hospital and as a Medicaid liaison for Elmwood Hills Care Center, Leon B. Howard, Jr. felt compelled to expand his service potential by accommodating the entire state of New Jersey as opposed to limiting himself to serving just the needs of those in the facilities that he so faithfully served. As the servicing needs of the community began to grow it was necessary for Project SHIFT, Inc. to hire more like-minded and qualified individuals to assist with this pursuit.


    Responsibilities:

    • Review transactions
    • Visit facilities to build partnerships to provide answers/solutions to individuals/agencies regarding specific social service concerns/issues

Krystale L. Ramsey

Krystale Ramsey has served faithfully for nearly 20 years as a case manager for Newpoint Behavioral Health and even after their merger with ACENDA. As Administrative Director for Project SHIFT, Inc., Ms. Ramsey is also our “Intake Coordinator.”

    As referrals from companies and individuals come in, Ms. Ramsey will assist in collecting and completing the service request forms which outline the specific need of the client and store the information onto our secure database for the consultant to access.   Ms. Ramsey also follows up on the clients and the consultant to assure that the process is running smoothly and on schedule. Ms. Ramsey also inquires updates from the agencies that are working on the benefits applied for to assure that nothing is held up on our end or on the client’s end to assure the process is complete and that the service requested has been accomplished.

    Responsibilities:

    • Intake. Taking new entries via phone and making calls to individuals/agencies
    • Interview individuals in need of services based on their social service needs
    • Complete applications for services needed
    • Record transactions on progress notes

Leona Howard

 Leona Howard is our Marketing Director.

Responsibilities:

  • Manage advertising distribution
  • Manage Demographic report of the number of individuals served per county NJ
  • Manage weekly/monthly/annual transaction reports
  • Present Tracker for Agencies under contract
  • Schedule outreach functions to reach people in need of services (quarterly)

Yvette Kennedy

 Yvette Kennedy is our Medicaid Assistance Manager.

  Responsibilities:

  • Interview clients eligible for Medicaid insurance and assist with completing their applications
  • Submit applications along with the DAR forms signed by client to represent them
  • Follow up on submitted applications and obtain documents from clients as needed to process the application
  • Advocate for client if client if denied the benefit by representing the client in Fair Hearing

TIFFANY MIXSON

Tiffany Mixson is our Housing Resource Manager
Responsibilities:

  • Take referrals in need of housing and provide housing options
  • To manage housing resources to include apartments/condos/shelters/boarding homes/nursing homes/rooming houses
  • To provide and apply individuals with subsidized housing options via government and private vouchers

Michelle Stroble

Michelle Stroble is our Outreach Worker/HMO Analysist

Responsibilities:

-Connect with social service agencies to include shelters, churches, and other nonprofit agencies in effort to collect information needed to complete SRF (Service Request form) for individuals in need of social service intervention keeping the Administrative Director informed.

-Planning and attending vending events with and for Social Service Agencies
-Coordinate and present working contracts with social service agencies
-Plan and coordinate courier services needed for individuals/agencies in need of documents requiring immediate completion and/or signature approval w/the Transportation Director
-Assist individuals in their selection of HMO and/or PPO plans and enroll the individual in the appropriate HMO/PPO and to manage any correspondence regarding their HMO/PPO plans.

Cheryl Faulkner

Cheryl Faulkner-Howard is our Welfare Assistance Manager Responsibilities: -Assist individuals with completing online applications for GA/TANF -Assist individuals with applying for TRA (Temporary Rental Assistance) for homeless GA candidates -Follow up on submitted applications and obtain documents from clients as needed to process the applications
